In my case, I had a recent build of Cyanogenmod based on Android 4.4.4, last updated in March 2015, and a modem firmware that was based on 4.2.2, last updated in May 2013.

Over time however, if you’re updating your OS but not your firmware, your cellular modem is getting gradually more and more out of sync with what the driver in the OS expects of it. Mostly these are incremental updates to improve stability or battery life. In the mobile phone world however, when Google updates Android, manufacturers update their firmware along with it. In any other device, I would expect the firmware to remain extremely stable, and new versions of the OS to include equally stable drivers. Turns out that just as Android gets regular updates, there are updates to the firmware of the cellular modem inside your phone.
